Missouri Transition Models of Success Submission Form

Missouri Department of Elementary & Secondary Education (DESE) in consortium with the Transition Coalition is identifying transition programs, practices, materials, instructional approaches, and curriculum that have shown to be effective in improving transition planning and services. Identified models will be shared across the state so local school districts can learn about new programs to replicate and enhance existing transition programs.

If you have a transition program or practice that has been effective in helping students prepare for life beyond high school, please share it with us by completing the form below. Your submission will be reviewed by a panel of transition experts, and we will contact you regarding your submission. It is important that you complete all of the questions below in order to be selected as a Missouri Model of Success.
