Your successful practices, materials and stories can be submitted by logging into your account on this website, going to the Missouri Models of Success Application and follow the directions for submitting your Missouri Models of Success. Please send them in!

Apply to be a Missouri MPACT Transition Mentor for Project STELLAR . The goal of the STELLAR project is to increase the ability of families to be informed and effective partners in their child’s transition from school to adult life and facilitate system change that supports youth with special needs to achieve their postsecondary goals in education, employment, living outcomes and community integration.

Transition Mentors are trained volunteers who provide support to families of children with disabilities throughout the transition process in their region of Missouri. You can download the Mentor Volunteer Description and the application.

Applications are due September 30, 2008.

This summer the Transition Coalition was busy updating and improving the Best Practices in Transition Planning online training module. It includes revised content, information regarding Indicator 13, and new resources in My Library.
